Modified FUE - Yaman Implanter®
When we take stock of the possible risk factors in hair transplantation, we can classify them under three groups;
1.Patient related risk factors,
2.Operation related risk factors,
3.Medical treatment and period of primary examination related risk factors.
With the new arrangements for hair analysis step in our clinic, now patients’ donor capacities and the recipient areas are evaluated much more effectively than before.
First of all, we analyze the capacity of donor area to define the exact number of graft which can be extracted from donor area by evaluating and measuring hair thickness, hair density on donor area, proportions of multiple follicles and area of donor. During this step, individual’s skin structure and possible skin problems are taken into consideration too. Later, we make measurement on the recipient area by considering hair thickness and proportions of multiple follicles and we calculate the exact number of graft which will be transplanted per square cantimeter on frontal, mid and crown areas. In the light of all calculations, individual’s expectaion and suitability for hair transplantation are determined before the operation. With detailed hair analysis we spot patient related risk factors and make the desicion on hair transplantation with exact calculations and measurements.
After hair analysis step, we prepare the patient for operation and accompany to the operating room. In our clinic, all of the operating rooms are arranged in conformity with the criteria of regulations of The Ministry Of Health, Republic of Turkey. The flor and the walls were built with an antibacterial material and sterilization of all operating rooms are under sutiable conditions. Operating equipments are sterilized in the criteria of The Ministry Of Health.
In the operating room, final preparations are completed. Frontal hair line is drawn symmetrically after measuring individual’s features. After that, operating area of patient is cleaned with antiseptic solution and local anesthesia is applied. Local anesthesia is applied with a dental neddle or depending on individual’s tenderness we can preanethesie with neddle free injector device. In the second methode, which provides an easier local anesthesia, anaesthetic is applied under the skin with the help of pressure without a neddle.
In our clinic, we perform incision as the first step of the hair transplantation. In the incision step, recipient incisions are created by Dr. Resul Yaman at once, in the exact number of graft calculation in front, mid and crown areas. The creation of recipient incisions is important for homogenization and hair roots’ directions. Incision step is performed with a 0.8-0.9-1mm. needles which differ in front, mid and crown areas depending on the proportions of multiple grafts. Also the implanter, which we have been developing lately, can provide us to create smaller incisions.
After incision step is over, we perform extraction. The extraction step is performed with a low speed modified micro motor by expert staff who has been trained only for extraction step. Individual’s donor area is divided into equipartition and from each partition same amount of follicle is extracted. In the donor area, hair follicles are extracted degressively to external line of the extraction area so an apparent external line is prevented.
Extracted hair follicles are waited in +4°C, hypothermosol solution which is used specifically in organ transplant surgeries. +4°C and hypothermosol solution provide us the optimal conditions for about 5 hours.
In the operations where is the possibility of that hair follicles may be waited outside for more than 5 hours, the operation is divided into two parts. Both parts are performed in the exact same day. With this methode, hair follicles don’t wait ouside long and we prevent a possible damage on the roots.
Transplantation step is performed with a spesific implanter which has been developed and taken out a patent by Dr. Resul Yaman. Different than classic implanter technique, in this spesific implanter hair follicles are taken from the upside of expert staff’s hand and they are pushed under the skin with the help of slice bar. The hair follicles are replaced under the skin without being touched by skin or forceps and by preventing any possible trauma which may be caused by crash or smash. So a possible damage on hair root, which may be caused by professional staff’s incidental mistake or causes making transplatation harder like hard skin structure, etc., can be prevented.
Different than classic implanter technique, in this spesific implanter, incision step is performed first so we aim to define hair roots growing directions correctly, the homogenization per square centimeter and the proportional hair density. In the operations, where the number of graft is high, implantation step is performed by two technicians. This may risk the homogenization and hair roots growing directions at right and left sides.
This risk is minimized by the methode where incision step is performed only by one person at the beginning of the operation. Also, because of that it is possible to create smaller incisions in the size of spesific implanter’s diameter than incisions which is created with forceps, this new implanter provide us denser transplantations. In our clinic, we minimize the operation related risk factors and prioritize surgical approachment for hair root protection.
Dr. Resul Yaman recommends his patient to use Finasteride or SawPalmetto-biotin during pre- and post-operative processes. Usage of medical treatments is considered as prior condition in under post-operative warranty.
FUE (Follicular Unit Extraction)
Hair transplantation is the only one scientific medical method that represents satisfied results in both men and women. Follicular Unit Extraction (FUE) is one of the method used in hair transplantation. FUE has become the most preferred technique in hair transplant surgery. In this FUE technique; hair grafts are taken circle cuts with 1 mm and smaller diameter equipments (motorised punch, manuel punch) on the scalp with hair. In FUE technique hair grafts are taken indivudially without doing linear cuts. FUE is the most preferred hair transplant surgery technique because of advantages and convenience. In addition to advantages it has also disadvantages.
The most important advantage of FUE is the high speed and to reach high number of grafts. For example; 1500-2000 grafts are taken at 1,5 hour for patients who has enough donor area with experienced doctor. It does not live a line mark on the operation area. Because small and circular touch is done on a big area and this prevents to live visible mark. With this advantage it is a choise for indivudials who use short hair. Using another part of body like beard, chest, leg as donor area easily is another advantage. By this way a lot of roots are getting and dense transplantation can be done on the working area. With the mostly used other technique FUT can not reach some areas but with FUE grafts can be taken from this areas. Also it is useful for indivudial who has keloid risk. Another advantage in FUE technique is not to use suture. After the operation small scar appears but this scar heal spontaneously without using suture or bandage. There is no pain on the donor area after taking the grafts. It provides flexibility to fix bad hair transplantation. Because of this special features FUE technique usage is increased and become important for indivudials who want to make hair transplantation. Every day new working types and innovations occured in FUE technique. It is simply to resolve this problem with healthy, safety methods. To prevent hair loss and search solutions was impossible with the technology in the past but today hair transplantation techniques are safety, healthy and exact way to prevent hair loss.
Eyebrow and Eyelash Transplantation
Single grafts removed by Fue method are used for eyelash and eyebrow transplantation. Double and triple grafts are separated and single grafts are made. Nape area is preferred as donor area. 200 to 600 roots will be enough for a single eyebrow according to gap on the eyebrow for eyebrow transplantation. The process after the procedure is same as hair transplantation. No water should be contacted for the first 24 hours. Transplanted hair follicles lose within 1 month. To apply massage along the growth direction of the eyebrows when they start to grow after 3 months will be effective for direction. First eyebrow strands will seem thick and hard; they naturally look like the hair of the nape area; eyebrows adopt the new area within a year and become thinner and have an eyebrow vision.
The important point in eyelash transplantation is whether the individual has healthy eyelids. If there is not any problem, hair roots taken from the nape area by local anesthesia are placed onto the eyelids through desired direction one by one. Because eyelashes transplanted from the hair roots will grow, they will need to be shorten in times.
50 to 100 roots may be enough for hair transplantation according to the need. Thinner hair roots are selected for eyelash transplantation from down side of the donor area.
Contrary to hair, beard and moustache transplantation, eyelashes may continue to grow without any loss in eyelash transplantation. Prices are not determined according to the hair root count ing eyebrow and eyelash transplantation. Pricing is performed according to the sessions.
